Transition Care Program

The transition care program helps older adults to regain their independence after they have completed a hospital stay. It provides short-term care for up to 12 weeks, and includes services such as social work, nursing, personal care and allied health services. State and territory governments are the approved providers of transition care and refer clients to non-government providers to administer different services. Under certain circumstances, approved providers may also apply for an extension of services but may require the client to undergo a range of clinical assessments prior to approval. Transition care services can be provided at a number of locations including but not limited to, a residential aged care (nursing) home, a person’s home, in the community or a mix of all of these.

1. What services does the program provide?

The program provides different kinds of support including but not limited to low-intensity therapy including allied health services such as physiotherapy, podiatry and occupational therapy, nursing support and also personal care services. Please note that Ritco exclusively provides allied health services under this program.

2. Who is eligible?

An assessment with an Aged Care Assessment Team (ACAT) member is required to determine eligibility. You may be eligible if you are an older person and are a patient in a public or private hospital, have been told that you are ready to leave hospital and you would benefit from receiving services for a short period of time.

If eligible, there is a limited and short time period leaving the hospital and commencing services.
